- Please advise the office staff when scheduling an appointment that your visit is due to an auto related accident.
- Please bring the following information to complete our Auto Accident Information Form which must be completed prior to your visit;
- Auto Insurance Name, Auto Insurance Billing Address, Policy Number, Claim Number, Phone Number, Accident Date and Time, and details.
SELF PAY PATIENTS
- We offer affordable self pay rates and payment arrangements in order to help you get your health care services. Please inquire about this information with the office staff when scheduling your appointment.
- We can also provide you with information on the NY Health Exchange to review your options for possible health insurance coverage.
